Healthcare services?
St. Johns County resident (proof required) age 18 years or older
Income at or below 300% of the Federal Poverty Guidelines (Family of 4 making less than $93,600/year)
No dental or health insurance, Veterans Administration (VA) benefits, Medicaid, or Medicare

What to bring to your first visit
Please bring a government or state-issued photo ID and proof of residency in St Johns County.
Proof of residency may include your driver’s license or photo ID, a letter from a local agency verifying your address, or a utility bill with your name and current address.
Please bring any pertinent medical records including a current list of any medications or supplements you are taking.
You may have them faxed to 904-824-1718 by a healthcare provider.
