Wildflower Healthcare offers FREE dental and medical services for adults in need in St. Johns County. Caring professionals deliver high-quality healthcare for qualifying patients regardless of race, gender, religion, or creed. We partner with physicians and dentists in the community to provide free specialty services when indicated. Additional services include prescription assistance, health screenings, healthcare education, and much more. We help to remove financial barriers to healthcare and strive to improve the lives of the people we serve.


Who can use Wildflower
Healthcare services?

  • St. Johns County resident (proof required) age 18 years or older

  • Income at or below 200% of the Federal Poverty Guidelines (Family of 4 making less than $55,500/year)

  • No dental or health insurance, Veterans Administration (VA) benefits, Medicaid, or Medicare

What to bring to your first visit

  • Please bring a government or state-issued photo ID and proof of residency in St Johns County.

  • Proof of residency may include your driver’s license or photo ID, a letter from a local agency verifying your address, or a utility bill with your name and current address.

  • Please bring any pertinent medical records including a current list of any medications or supplements you are taking.

  • You may have them faxed to 904-824-1718 by a healthcare provider.
